Problematic definition of ‘COVID-19 death’ may be inflating death rate, leading to draconian lockdowns

April 1, 2020 (LifeSiteNews) – Some have questioned whether the extreme measures taken to attempt to reduce the spread of “Covid-19” may be doing more harm than good. One question that arises is: upon what facts are government entities basing their decisions to lock up entire states and cancel the public celebration of the Most Holy Sacrifice of the Mass?

Decisions throughout America are likely based, at least partially, upon the tally of “Covid-19 deaths”. It is a somewhat ambiguous phrase, and when one performs the research, one likely finds that there does not appear to be a clear universal definition of a “Covid-19 death.”

In the U.S., the Center for Disease Control and Prevention (CDC) is the authoritative source for a definition. But a definition does not appear to be explicitly stated in expected locations; the CDC’s Covid-19 cases and deaths statistics’ page does not provide a definition of “Covid-19 death.” Does the phrase only refer to deaths from severe acute respiratory syndrome solely caused by the SARS-CoV-2 virus? Or does “Covid-19 death” refer to all deceased people who have received a positive test for the SARS-CoV-2 virus? Does it refer to some who have been merely assumed to have died indirectly from Covid-19-like complications? Does the “Covid-19 death” tally include deceased who had co-infections with other viruses which can cause severe acute respiratory symptoms or pneumonia like H1N1, influenza A or B, or respiratory syncytial virus and thus could also be tallied as a “swine flu/H1N1 death” or “influenza death”?

Are some “Covid-19 deaths” resulting from easily preventable conditions like dehydration rather than severe acute respiratory syndrome?

Most commentators have focused on comparing Covid-19 to influenza, but respiratory syncytial virus can also cause pneumonia and results in an estimated 177,000 hospitalizations and 14,000 deaths in the U.S. each year. Can a “Covid-19 death” also be the result of a co-infection with respiratory syncytial virus (RSV) and therefore be labeled as an “RSV death”?

Additionally, the CDC’s Covid-19 tally includes “presumptive positive cases of COVID-19 reported to CDC” (added emphasis). Does the tally of “Covid-19 deaths” also include “presumptive positive cases of Covid-19”? Are states and the federal government basing decisions to prohibit public celebration of the Most Holy Sacrifice of the Mass, impose unemployment, potential poverty, and potentially life-threatening problems on millions of Americans off of faulty and misleading assumptions and presumptions?

Again, it is not outright evident what is included in the phrase “Covid-19 death.” One has to do some literature research on the source of mortality data for potential hints. According to the CDC, “[t]he basic source of information about mortality is the death certificate.” One clue, then, is observable in how the CDC is advising physicians to fill out death certificates. Death certificates should describe immediate cause of death as well as factors contributing to death. In many states, death certificates are public records. From the CDC:

Death certificates are used for surveillance [of HIV/AIDS, influenza, pneumonia, tuberculosis, etc.], outbreaks, and emergency activities…because death is a sentinel event. Rapid reporting of deaths is required by law. The information is used at local and state health departments and the Centers for Disease Control and Prevention (CDC) for rapid response. (No. 6)

It is apparent, then, that national mortality data including the CDC’s “Covid-19 deaths” statistics arise from what is reported on death certificates.

Physicians document the medical information on the death certificate. How, then, are physicians advised to record a death in which SARS-CoV-2 virus or “Covid-19” may be involved?

The CDC recently published a “Covid-19 Alert” dated March 24, 2020 with guidance on the topic. The document summarizes potential questions from death certificate certifiers, which are most often physicians. One question which indicates how broadly “Covid-19 death” is being defined is, “Should ‘COVID-19’ be reported on the death certificate only with a confirmed test?” The CDC gives the following guidance:

COVID-19 should be reported on the death certificate for all decedents where the disease caused or is assumed to have caused or contributed to death. Certifiers should include as much detail as possible based on their knowledge of the case, medical records, laboratory testing, etc. If the decedent had other chronic conditions such as COPD or asthma that may have also contributed, these conditions can be reported in Part II. [Original emphasis]

If Covid-19 is “assumed to have caused” or “assumed to…have contributed to death”, it is to be reported on the death certificate. “Covid-19 caused death” and “Covid-19 is assumed to have caused or contributed to death” are two very different descriptions; yet, they are apparently included under the same phrase of “Covid-19 death.” Furthermore, as it is written above, a deceased may be included as a “Covid-19 death” even if the deceased was not confirmed to have had Covid-19.

It is known that many “Covid-19 deaths” occur in individuals with multiple underlying medical problems. And if a “Covid-19 death” includes “decedents where the disease caused or is assumed to have caused or contributed to death”, as the alert above states, then it follows that the CDC’s official tally of “Covid-19 deaths” does not mean what millions of Americans think it means.

“Covid-19 deaths” also apparently includes deceased who may have had co-infections with other viruses which also could be “assumed to have contributed to death”. Examples include HIV/AIDS which weakens the immune system along with non-Covid-19 viruses which may cause pneumonia and severe acute respiratory symptoms. How many “Covid-19 deaths” are among persons with multiple viral infections?

One could assume that SARS-CoV-2 virus would contribute to death in a patient who already has pneumonia resulting from H1N1. Or, one could assume there are deceased who have tested positive for co-infections with the SARS-CoV-2 virus, the swine flu, influenza, and/or respiratory syncyctial virus. According to the above federal government guidance to physicians, the death certificate of such deceased who tested positive for SARS-CoV-2 virus along with other viral infections would be counted in the CDC’s tally of “Covid-19 deaths.”

A related statistic that does not appear to be publicized is the number of “Covid-19 deaths” among immunodeficient persons. A reasonable person might ask if the deceased recorded as “Covid-19 deaths” contracted respiratory syncytial virus or influenza instead of SARS-CoV-2 virus, would they still likely die? And if those are included in the “Covid-19 deaths”, should mortality data of those deaths be the basis for shutting down almost the entire country? How many “Covid-19 deaths” are among working-aged or working-class persons without immunodeficiency? This is not to trivialize life and death but instead to question possibly problematic logic in decision making among powerful government authorities.

The CDC’s alert above indicates that “Covid-19 deaths” includes some who may not have even tested positive for the virus. Other “Covid-19 deaths” may include mere assumptions that Covid-19 probably contributed to death in persons that may have succumbed to other viruses had they contracted them.

Notably, too, the above guidance on how Covid-19 deaths should be reported by physicians on death certificates is dated March 24, 2020. The alert provides a very broad guidance of what physicians should report on death certificates and is then apparently included in the CDC’s tally of “Covid-19 death” mortality data. If one views the data of daily reported deaths after March 24, 2020, one finds that there was an apparent sudden increase in daily “Covid-19 deaths.” This may not be due to spread of the virus, though, as one may be misled to think. It may be a result of the March 24, 2020 guideline from the CDC which says that “COVID-19 should be reported on the death certificate for all decedents where the disease caused or is “assumed to have caused or contributed to death”; as this is stated, it would include some that have not tested positive for SARS-CoV-2 virus. And, as written, it includes many others that may have co-infections with other viruses which may be “assumed to have contributed to death”. Or, it could include patients with COPD and a recent history of pneumonia resulting as a side effect of certain medications but with Covid-19 “assumed to have contributed to death”. Thus, mortality statistics of “Covid-19 deaths” could have immediately inflated after March 24, 2020 as a result.

And, the CDC itself notes that it is often the case that deceased individuals have “so many medical problems” which make it difficult to specify the underlying cause of death on a death certificate. Apparently in reference to such circumstances, the CDC guides in the following manner:

the rules for coding and selection of the underlying cause of death are expected to result in COVID19 being the underlying cause more often than not.

Since all patients testing positive for SARS-CoV-2 virus will already likely be reported to the state and local health departments as well as the CDC, and since the CDC is nudging physicians to include Covid-19 as the underlying cause of death, it seems likely that physicians will “more often than not” include this on the death certificate just to be safe in terms of preventing possible future legal action.

Oftentimes Catholics get falsely accused of opposing science. Here we have a case of a powerful government entity using assumptions, “probables”, and pre-determined or skewed results which may inflate mortality data. That is in opposition to basic scientific principles. If drug study leaders (with trillions of dollars to potentially give to hospitals) comparing two different drugs informed all physician participants throughout the U.S. to expect to report Drug A to be effective or to report “more often than not” Drug B will be the underlying cause of death, the results of such a study would be considered at minimum not trustworthy.

Indeed, the “Covid-19 deaths” tally does not appear to be defined in the way that most people think. And it appears as though some of the most extreme decisions in the history of America may not be based upon scientific evidence but instead on projections, assumptions, probables, and possibly inflated mortality rates.

The federal government is classifying the deaths of patients infected with the coronavirus as COVID-19 deaths, regardless of any underlying health issues that could have contributed to the loss of someone's life.

Dr. Deborah Birx, the response coordinator for the White House coronavirus task force, said the federal government is continuing to count the suspected COVID-19 deaths, despite other nations doing the opposite.

"There are other countries that if you had a pre-existing condition, and let's say the virus caused you to go to the ICU [intensive care unit] and then have a heart or kidney problem," she said during a Tuesday news briefing at the White House. "Some countries are recording that as a heart issue or a kidney issue and not a COVID-19 death.

"The intent is ... if someone dies with COVID-19 we are counting that," she added.

Asked whether the numbers could skew data the government is trying to collect, Birx said that would mostly apply more to rural areas where testing isn't being implemented on a wide scale.

"I'm pretty confident that in New York City and New Jersey and places that have these large outbreaks and COVID-only hospitals. ... I can tell you they are testing," she said.

Dr. Michael Baden, a Fox News contributor, said it's reasonable to include the death of someone infected with the virus, who also had other health issues, in the COVID-19 body count.

"In the normal course, autopsies would then determine whether the person died of the effects of the COVID virus, whether the person had a brain tumor or brain hemorrhage for example that might be unrelated to it and what the relative significance of both the infection and the pre-existing disease is," Baden told Fox News.

However, the number of autopsies being performed could be low due to the danger of infection, he said.

"Then you will include in those numbers some people who did have a pre-existing condition that would have caused death anyway, but that's probably a small number," Baden said.

As US coronavirus death toll mounts, so does the belief by some that it is exaggerated

There’s a reason why some people believe government officials are exaggerating the number of COVID-19 fatalities.

One problem is the hodgepodge way states tally those numbers, Fox News has found.

Some states count presumed coronavirus deaths along with confirmed cases under Centers for Disease Control and Prevention guidance issued last month. Other states don’t count those deaths.

Deaths have been classified as a COVID-19 death even after a physician or loved ones reported otherwise. And those who died “with" COVID-19 have been included in the count with those who died “of" COVID-19.

“I think a lot of clinicians are putting that condition (COVID-19) on death certificates when it might not be accurate because they died with coronavirus and not of coronavirus,” Macomb County, Mich., Chief Medical Examiner Daniel Spitz in an interview with the Ann Arbor News last month.

“Are they entirely accurate? No,” Spitz said. “Are people dying of it? Absolutely. Are people dying of other things and coronavirus is maybe getting credit? Yeah, probably.”

The doctor also told Fox News that he believes there are people who died of COVID-19, but weren't counted. Determining a COVID-19 death is based on each doctor's best clinical impression and that varies, he said.

The debate over whether the COVID-19 death count has been exaggerated has intensified as deaths from the virus continue to rise at a steady and alarming rate.

As of Friday, there were 85,974 deaths due to COVID-19 in the United States, according to Johns Hopkins University data. The CDC was reporting 83,947 COVID-19 deaths on Friday.

On Wednesday, a report said President Trump and members of the White House Coronavirus Task Force are pushing the CDC to revise the way it counts coronavirus deaths. That change could lead to far fewer deaths being counted.

Trump has privately questioned the number of COVID-19 deaths as the death toll surpassed the 80,000 mark this week, suggesting it may be incorrect or inflated by the current methodology, the Daily Beast reported.

Three administration officials said Deborah Birx, the task force response coordinator, has urged the CDC to exclude from the death count some who were presumed infected, but did not have a confirmed lab result and those who had the virus but might not have died as a direct result of it, according to the news outlet.

COVID-19 DEATH NUMBERS GENERATE DEBATE OVER ACCURACY

Last week, The Washington Post reported that at a recent discussion on COVID-19 data, Birx told CDC Director Robert Redfield that “there is nothing from the CDC that I can trust.”

According to the paper, Birx and others feared the CDC was inflating coronavirus statistics, like mortality rates and case numbers, by up to 25 percent.

But not everyone shares the view that COVID-19 deaths are being overcounted.

Dr. Anthony Fauci, the top U.S. infectious disease expert, said during testimony before the Senate Health, Education, Labor and Pensions Committee Tuesday that he believes the coronavirus death toll is “almost certainly higher.”

He pointed to the situation in New York City at the peak of the outbreak. New York now has more than 27,000 COVID-19 deaths.

“That there may have been people who died at home who did have COVID, who were not counted as COVID because they never really got to the hospital,” Fauci testified.

And other experts have said the COVID-19 death toll hasn't captured all those who died of the virus because there still isn't enough testing being done, especially in nursing homes, where the virus attacks vulnerable elders with underlying medical conditions.

Additionally, the CDC found Monday that the number of excess deaths in New York City during the pandemic suggests an undercount of 5,000 coronavirus deaths.

Last month, Factcheck.org debunked claims that hospitals were inflating the number of COVID-19 cases and deaths so they can be paid more from Medicare.

Those claims arose because hospitals can charge Medicaid and Medicare an extra 20 percent for treating a COVID-19 under the CARES Act approved by Congress to deal with the coronavirus pandemic.

“There’s an implication here that hospitals are over-reporting their COVID patients because they have an economic advantage of doing so, [which] is really an outrageous claim,” UCLA senior fellow Gerald Kominski told the fact-checking site.

He said any suggestion that patients may be put on ventilators out of financial gain, not medical need, “is basically saying physicians are violating their Hippocratic Oath … it would be like providing heart surgery on someone who doesn’t need it.”

Hospitals have another motive not to inflate COVID-19 death numbers. If the numbers don’t add up, they could expose themselves to Medicare audits, and civil and criminal penalties.

Medicare also encourages whistleblowers to report suspected wrongdoing by offering financial rewards.

Illinois has been accused of skewing its COVID-19 death count. The CDC on Friday reported 3,792 coronavirus deaths in Illinois, which has been hard hit by the pandemic.

In April, the state’s Director of Public Health Dr. Ngozi Ezike told a coronavirus news briefing that the state’s COVID-19 death toll was counting those who died of “a clear alternate cause” but had the virus at the same time.

“So, everyone who's listed as a COVID death doesn't mean that that was the cause of the death, but they had COVID at the time of the death,” she said.

"You could be driving home and be in an auto accident and die from that," Darren Bailey, a Republican Illinois lawmaker suing Gov. JB Pritzker to reopen the state, told WAND-TV on Monday. "And if you had COVID-19 in your system, that death gets labeled as COVID."

The next day, Ezike offered clarification. She told reporters that Illinois was not counting those whose COVID-19 infection had nothing to do with their death.

“We are trying to make sure that things that aren’t related at all to the COVID diagnosis are removed, but if someone has another illness, like heart disease, and then had a stroke or other event, it’s not as easy to separate that and say COVID didn’t exacerbate that existing illness. That would not be removed from the count,” she said.

Tennessee doesn’t count presumed COVID-19 deaths; it had 273 COVID-19 deaths as of Friday.

The state’s health department said it tracks those deaths, but isn't reporting them.

“Some of those pan out to be actual cases or some of them don’t,” state Health Commissioner Lisa Piercey told reporters at a coronavirus briefing on April 29.

“We collect that information and have actually had significant discussion on whether or not to post that information,” she said. “Have no problem doing it. We just don’t see that it adds a lot of value until we actually have a confirmed case or not.”

Colorado counted a man who a county coroner said died of acute alcohol poisoning as a COVID-19 death.

Montezuma County Coroner George Deavers told the Durango Herald the man’s blood-alcohol level was 0.55, or almost seven times the legal driving limit of 0.08 in Colorado. A BAC of 0.3 is considered lethal.

“COVID was not listed on the death certificate as the cause of death,” Deavers said, the paper reported Wednesday. “I disagree with the state for listing it as a COVID death, and will be discussing it with them this week.”

News reports have identified the man as Sebastian Yellow, 35, and reported that he was found dead by police May 4.

The Montezuma County Public Health Department also was refusing to report Yellow's death as a COVID-19 death. “The state is reporting that death as a COVID death, but our health department wanted to let people know that even though the person did have the virus, they did not die from it,” the agency said.

In response to a request for comment about Yellow’s death, the Colorado Department of Health told KCNC-TV that it classifies a death as confirmed when there is a positive SARS-CoV-2 (COVID-19) laboratory test.

Last month, the same agency reclassified three deaths at a Centennial nursing home as COVID-19 deaths, challenging the findings of attending physicians who ruled the deaths were unrelated to the virus.

The deaths occurred at Someren Glen, where four other residents died of COVID-19. The state has now recorded all seven deaths as COVID-19 deaths.

“We have never seen a situation where the health department overrules a physician’s findings,” Tim Rogers, the facility’s executive director, told KCNC. “However, these are unprecedented times and the health department official did not share their motivation for changing physician’s orders.”

A health department spokesman told the station of those deaths that the agency was following CDC guidance.

“When a person with a lab-confirmed case of COVID-19 dies, their death is automatically counted as a COVID-19 death unless there is another cause that completely rules out COVID-19, such as a fatal physical injury,” the spokesman said. “A pre-existing illness would not rule out COVID-19.”

Alaska’s COVID-19 death toll rose to 10 last week with the death of 90-year-old Donald VanBuren.

But despite testing positive for the virus, VanBuren, who lived alone, was looked after by neighbors who said his death was from cancer and kidney failure, not COVID-19, according to MustReadAlaska, a news blog.

Sarah Vance, a Republican member of the Alaska House of Representatives, sent a letter to Alaska’s chief medical officer Anne Zink demanding an explanation.

“People want accountability and answers because there is so much confusion and upset in our daily lives with the coronavirus,” Vance told Fox 4 Anchorage on May 11.

Zink replied that “when we have a case that is positive and a person that has passed away our process is to go to the clinician and asked specifically if they think this was related. If they say yes, then we report it out to you.”

In Arkansas, where 97 COVID-19 deaths have been reported, the count includes a woman who died at a nursing home in April.

But KTHV-TV reported interviewing the woman’s daughter, who had questions about including her mother in the count.

"My mother was 91 when she passed,” the daughter, who requested anonymity, said. “She had been in hospice since January. I feel the diagnosis of 'respiratory failure due to COVID-19' is not true. To add a death due to the virus just to inflate the numbers is not right."

She added, "I could see why people would not want to consider that as the primary cause of death. That person was already frail, near the end of their life and they were in hospice for other reasons, so this was likely a complicating factor. But likely, the other reason would have caused the death."

In Virginia, there have been 955 COVID-19 deaths. The count includes Garry Lee Burner, 63, of Woodford, who died May 7 at home.

But Burner’s daughter told a newspaper Wednesday she believes heart failure ultimately caused his death — he’d had a major heart attack eight years ago, followed by mini strokes.

Burner worked until he got sick in April. He was hospitalized, then discharged with an oxygen tank. A second test in the hospital confirmed he had the virus.

“I just can’t stand the fact of people saying he died from this virus,” Robin Burner told The Free Lance-Star in Fredericksburg. “Yes, he had it and it probably made him weaker, but in my heart, I don’t think that’s what he died of. I think he had a heart attack.”
